Then someone from the office will contact you to book a virtual or in-person visit.
Virtual visits can be done by Zoom, Facetime, Skype, by phone, or occasionally even by Facebook. I mostly use phone visits or Zoom for Healthcare (because it has enhanced security )
Hit the "SUBMIT" above when you have filled out the form, and it will get forwarded to the office.
Then someone will get in contact with you to arrange an appointment, a phone appointment, or a Zoom appointment.
(For Zoom, you don't need the program, but you do need an email. We can handle the technical details. This web site and Zoom for Healthcare have security similar to sites where you give your Visa etc to buy things, so everything is very confidential.
